Fill out the .pdf file then save it onto your hard drive than e-mail it to tjackvideo@yahoo.com. 2. You may also fill out the wedding form online and submit it. A confirmation page will be sent to you automatically as soon as the form has been successfully submitted and an e-mail will be sent to the e-mail address provided.
